TELEHEALTH DISCLOSURE

WHAT IS TELEHEALTH?
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. Telehealth may be used for diagnosis, treatment, follow-up, and patient education.
THREE-ENTITY MODEL
The READY platform operates under a three-entity model:
- READY — A telehealth technology platform that facilitates connections between patients and healthcare providers. READY is not a healthcare provider and does not practice medicine.
- Telehealth Provider — An independent licensed medical group that provides clinical services, including reviewing patient intakes and issuing prescriptions where clinically appropriate. The Telehealth Provider operates independently of READY.
- Pharmacy Partner — A licensed 503A compounding pharmacy that compounds and dispenses prescription medications pursuant to valid prescriptions. The Pharmacy Partner operates independently of READY.
PATIENT RIGHTS IN TELEHEALTH
As a patient receiving telehealth services, you have the right to:
- Withhold or withdraw consent to telehealth services at any time without affecting your right to future care.
- Inspect all information obtained and recorded during a telehealth interaction.
- Be informed of all persons who will be present during a telehealth consultation.
- Receive a copy of all medical records pertaining to your telehealth consultation.
LIMITATIONS OF TELEHEALTH
Telehealth services may not be appropriate for all medical conditions. The Telehealth Provider may determine that an in-person evaluation is necessary and may decline to prescribe via telehealth. In the event of a medical emergency, call 911 or go to the nearest emergency room.
COMPOUNDED MEDICATIONS DISCLOSURE
DEEP BREATH™ is a compounded prescription medication. Compounded medications are not FDA-approved and have not been evaluated by the FDA for safety, efficacy, or quality. Compounded medications are prepared by a licensed 503A compounding pharmacy pursuant to a valid prescription from a licensed healthcare provider. A prescription is required.
